What is Regenerative Medicine?

Regenerative medicine is a general broad category that includes treatments such as stem cell therapy, platelet rich plasma (PRP) therapy, prolotherapy, certain cartilage regeneration procedures, and cellular therapies. It can be useful when an injury is unlikely to heal with traditional non-surgical treatments, such as rest and physical therapy, or pain and function have not improved following rehabilitation or surgery. By combining these approaches, we can boost your natural healing process and help restore normal function.

How Does Regenerative Medicine Work?

The goal of regenerative medicine is to replace or reboot tissues or organs damaged because of disease, injury, age, or other issues instead of treating symptoms with medication and procedures. Combinations of these approaches called regenerative injection therapy (RIT) can amplify our natural healing process with the goal of:

  • Inhibiting inflammation and slowing down the progression of degeneration
  • Inducing repair to stimulate the formation of new cartilage or tissue.
  • Reduce pain by altering a patient's pain receptors and pain sensation processing
  • Restore homeostasis

RIT Basics

Some of the options available include:

Prolotherapy (proliferate + therapy) : Prolo is a blanket term used to describe what happens when we inject irritating substances such as dextrose into the tissue. It tones and tightens up weakened tissues to help with range of motion and pain relief and can be highly effective in osteoarthritis relief.

PRP : Platelet-rich plasma therapy or PRP is a treatment that attempts to take advantage of the blood's natural healing properties. Platelet-rich plasma is taken from a sample of the patient's blood it the office and processed to concentrate plasma. The therapeutic injections contain a higher concentration of platelets than is found in normal blood. Platelets secrete substances called growth factors and other proteins that regulate cell division, stimulate tissue regeneration, and promote healing.

Tissue Matrixes: Utilizing cellular modulation, our body can use stem cells as a way of repairing itself.

  • Cord Blood contain a high percentage of exosomes that can restore cells throughout your body. They enhance cell-to-cell communication, which is essential for overall cellular health. They also can promote differentiate into more specialized cells. Compared to adult stem cells, exosomes contain nearly three times the amount of growth factors
  • Wharton's Jelly is classified as a connective tissue, it is abundant in extracellular matrix, composed of glycosaminoglycans (mainly hyaluronic acid), as well as different types of collagens. It is a type of mesenchymal stem cells (WJ-MSCs) which are a class of stem cells with high differentiative potential.
  • Vitti-Pure Vs PRP
    VITTI-PURE uses umbilical cord tissue to offer a support matrix abundant in growth factors for joints, ligaments, tendons, and other musculoskeletal applications, helping repair in the patient. Peptide Therapy : Peptides are chains of amino acids that act as cell signaling molecules. Regenerative therapies are paired well with specific peptides that act to induce a variety of physiologic activity such as cell turnover, hormone modulation, muscle growth and more

Repair/Healing Peptides:BPC 157 was discovered in 1993, and there is now considerable clinical data of the regenerative properties of BPC-157 in animals. Human studies are currently underway. Uses for BPC-157 include reducing inflammation, increasing the production of type 1 collagen, improving the lower esophageal sphincter, healing fistulas, and helping with repairing many parts of the body. Published studies of BPC-157 will be reviewed, as well as dosing. This peptide pairs well with multiple others particularly Thymosin B4

Peptides for Pain:There are many potentially useful peptides for pain management depending on the primary issue and degree of secondary contributions. In pain management it is often useful to stack and rotate various peptides to address multiple issues simultaneously.

Useful peptides for pain include Thymosins, ARA290, BPC157, pentosan polysufate and a unique peptide derived from venom, melittin.

Alpha 2 Macroglobulin (A2M):

 This is a cutting-edge new treatment for osteoarthritis and other painful orthopedic conditions. A2M is a human plasma protein molecule that occurs naturally in your bloodstream and when injected into the osteoarthritic or painful area, begins to promote healing.

When a patient is suffering from arthritis, chemicals that destroy cartilage affect his or her joints. Catabolic proteases, the chemicals cause cartilage to break down, result in pain and inflammation. The disease will eventually cause the cartilage to disappear.

A2M halts the progression of osteoarthritis at the molecular level by inactivating the chemicals that cause the joints to breakdown. As the A2M captures and neutralizes the damaging chemicals, the body will work to eliminate them. The joint will then begin to recover, as the inflammation is subdued. This inhibiting factor makes A2M a viable option for biologic treatments and promotes increased strength, and improves overall function. This is called the healing cascade, which can be active and take place over a 4-6 week period.

This is typically done as a 2 injection series for joints. The first with A2M and the second with MSM And Whartons Jelly. Patients also have the option to add regenerative peptides such as BPC157 which further promote healing.

BPC-157 (Body protection compound-157), is a stable pentadecapeptide first derived from gastric juices. It promotes microscopic regeneration; improves tissue granulation, fibroblast recruitment and collagen formation. In laboratory studies BPC-157 has been reported to accelerate the healing of wounds, injuries and tissues, including tendons, ligament, bone, muscle, skin and gastric mucosa. In addition BPC157 has broad effects to include:

  • Modulates angiogenesis (increased blood vessel formation)– increased when needed, decreased when needed such as in tumor metastasis.
  • Stimulates NO (nitric oxide) production which is important for circulation.
  • Upregulates anti-inflammatory gene transcription factor
  • Downregulates TNF-alpha – anti-inflammatory
  • Upregulates growth hormone (GH)

In addition, it is known to be cardio and neuro protective.

MSM is rich in organic sulphur, an important ‘building block’ for healthy bones and joints, and it’s very useful for your immune system. Available evidence from short-term RCTs shows that MSM may have a moderate effect in improving joint pain and swelling as well as general functional wellbeing in people with osteoarthritis. Laboratory studies have found that MSM has anti-inflammatory and antioxidant effects. Sulphur, which is a major component of MSM, plays an important role in making collagen and glucosamine, both of which are vital for healthy bones and joints, and in the production of immunoglobulins, which help your immune system.

A2M is from your own blood, autologous, so there is little to no risk when conducted by a trained professional. Since the cells are autologous there is no chance of having an allergy or immune reaction. Side effects of complications of A2M are extremely rare. MSM has the toxicity equivalency to water and is safe for people with “sulfa” allergy and BPC157 has not been found to have any toxicity even at extremely high doses.
